Insurance companies typically have a paper form or online submission process for you to claim OON reimbursement. No worries because we provide you with a Superbill – a document to help you claim reimbursement from your insurance company if your plan includes coverage for Out Of Network behavioral health or mental health services.

What’s a Superbill?

A Superbill is a Statement for Insurance Reimbursement that includes:

  • Client Details: Personal and insurance information.

  • Provider Information: Therapist’s credentials and contact.

  • Practice Information: Unity Collective’s practice details.

  • Diagnosis & Service: Dates, codes, and fees for services rendered.

Common Service Codes to Check for Coverage On

  • 90834: 45 min psychotherapy.

  • 90837: 60 min psychotherapy.

  • 90846 & 90847: Family therapy sessions.

  • +90836: Extended sessions.

Note: Telehealth sessions are marked with POS Code “02”.

How It Works

  1. Pay at Service: Use your Credit Card, including HSA/FSA.

  2. Receive Your Superbill: Opt-in for monthly automated Superbills.

  3. Submit to Insurance: Use it to discuss coverage with your insurer.
